We coordinate and manage non medical in home caregiving for individuals on hospice or for families who want their loved one to remain at home. We oversee staffing, scheduling, and care coordination, and work closely with hospice teams, physicians, and family members to ensure continuity, safety, and clear communication.
Structured for families who require consistent, high-acuity support without gaps in care. All shifts are guaranteed with no call-offs, supported by active care management and oversight.
Rates range from $37 to $48 per hour, based on total monthly care hours and level of support required. Higher monthly hour commitments qualify for lower hourly rates.
*All holistic end-of-life services are included and integrated within this model of care.
ForeverLinkT™ is a structured end-of-life planning service that supports aging caregivers and ensures continuity of care for adults with complex needs.
Rate $135 hr
Estimated Timeframe: 30 to 50 hours, depending on the complexity of needs
Estimated Total Cost: $4,050 to $6,750
While grief support is included in all time block packages, stand-alone sessions are available for continued or additional support as needed.
Rate $115 per session
Session between 60 to 90 min
Offered outside of time block packages, this 90-minute guided practice supports deep rest, nervous system regulation, and non-pharmaceutical pain relief.
Sessions are $130
